okay sorry my internet and kchan image server is faster than 3 years ago so haven't really thought about this stuff since then
it wasn't about hovers but image popups you click open (popup backdrop off). let's say if you click an image it gets stored in popup slot #1, then you close it and click another image. the old #1 is shown first until new image is fully loaded
now let's say you click two images open, #1 and #2. you close both and then try open two other images, now you see old #1 and #2 until new images load.
so if you at some point had say 3 images open, that #3 slot may have been stored for a long time until you finally have 3 images open and get to see it again. At that point it may indeed seem super random.
you can check this out by throttling to 2g or 3g speeds, quite interesting at first I thought it was some Preact thing but it only seems to happen on Firefox, not Chrome. So maybe it is indeed not a security risk?
